There will be fallouts, but for the targeted city's area only. I could make it so that others suffer as well, but I don't know if I'll have the time.
If others would to suffer, the EHE would take that into consideration and try to solve this "problem", or possible problem. But the effect of a nuclear strike aren't just in the fallouts. The initiator will surely have big political (and military) strikebacks and it's population will surely contest. These consequences are important enough to make the EHE think twice before launching.
A nuclear strike will probably be always last resort in Defcon for the EHE. The consequences are so huge that it will need to be the last resort. I guess that's what it should be anyway.
